USSF Requirements for US Citizens Born Abroad
Find the row that matches your player's circumstances. Print and complete the required document and submit it with the additional required items as indicated on the Wake FC website. Documents must be in 1 of the 4 languages that FIFA accepts: English, Spanish, German, French, or accompanied by an official translation in one of the accepted languages.

Age: 0-17 Living in US before turning 10 years old | P-10 Form (P10 3-15) | Proof player entered US before 10: Report card, baptism, immunization record, previous record of playing another sport; Document MUST have player's name and the date. |
Age: 10-17 Entered US after turning 10 years old Did NOT play for a foreign club | First Registration (FR-11) | Proof of citizenship: Passport OR Birth Certificate |
Age: 10-17 Entered US after turning 10 years old Did play for a foreign club | International Clearance Form (ITC 5-11) | Player may NOT play with club until USSF contacts us with approval. Same supporting documents required as described in block above. |
